Option 1: Download forms (Registration, medical and waiver) from web
site under registration packet link. Complete forms and mail with deposit
or call to make deposit by credit card. We need these forms returned
no later than 45 days before course starts.
Option 2: Call or

we will mail or email the registration packet. We can take credit card
numbers via phone for deposits and final payment. We need these forms returned
